Individuals wishing to salute their own or loved-one's military service are invited to fill out the form provided. A bronze plaque four inches high by nine inches wide will be locally forged and then mounted in the area of your specific service. Each plaque may contain four lines of up to 22 letter spaces each for additional unit or medal information. Plaques are available for a tax deductible donation of $300.
